[Problem] Multi PVC annex A

Sorry for my English.
I have SX-541 with firmware 2.15 annex A.
My ISP have 3 VPI/VCI:
1 0/40 for Internet
2 0/41 for VOIP
3 0/42 for IPTV
I have adjusted connections only for the Internet. Work fine.
Only fixed telefon network Works not always.
Now I need adjusted connections for IPTV in LAN4 and adjusted connections for the Internet in WIFI & LAN1,2,3. Can you help me?
