VoIP quality problem when internet is in use

Hi everyone, sorry for posting in english.
I've got DSL connection @ 3Mbit down / 768 Kbit upstream.
It is stable and VoIP works flawlessly when internet usage is "regular" - I mean, active websurfing, bittorents etc.
Prioritization set is mostly by default - VoIP has realtime priority, websurfing has "normal" and bittorents set to run in background.
Whenever internet usage growth to 100%, I'm facing VoIP quality impacts like heavy distortions, delays. That especially true when I'm using TV streaming (not IPTV, but HTTP and MMS traffic).
Tried to lowered priority for that TV traffic, even to background, but cannot get rid of the problem.
Online Monitor shows upstream not used to 100%, so maybe that's the problem, FB probably "doesn't see" that lack of downupstream and cannot correct that?
Any suggestion really welcome, thanks.

UPD: is there any way to "reserve" some portion of downstream for VoIP only?

lower the simultaneous connections of bittorrent or stop it.
Thought about it too. Torrent client disabled now, but that didn't change situation.

Your speed is anyway too low, sorry.
Too slow for what? AFAIK, VoIP requires about 192-256 Kbit/s. Bandwidth reservation could be a solution for me, because at least 512K down/up never usually not used, as it shown in the Online Monitor.

actually, tried to reset prioritization settings already, but ok, thank you for advices, will try that once again.
 
So, played a bit more with firmware.
First of all, removed freetz and made a clean install of 04.82 firmware.
Then, reset the internet settings to defaults:
(see attachments - 1st and 2nd)
Tried a 1 min 40 sec VoIP call (nonoh.net) - quality was really poor. 3rd attachment.

Stopped the TV streaming and called again to the same phone - quality became excellent.
.
Changed account settings - limited upstream to 320 Kbit and downstream set at 3200 Kbit.
Called again - 2 min call - last attachment. No changes, quality was ugly.
.
My ISP povides a special fee-based option to temporarily raise a downstream.
Raised downstream to 5 Mbit. Upstream remains the same @ about 768 Kbit.
Downstream utilization, aswell as upstream stayed at the same level (a bit less than 3 Mbit down / 100 K up), but VoIP still very very very poor.

So, I ran out of ideas... Pls advise further! Thank you!

So, about 2 months of talks with AVM support passed. During that, I found the problem doesn't refer neither to DSL connection (tried to change ISP), nor to regular downloading, nor even to prioritization settings.
With regular downloads, there is no problem with VoIP, only when MPEG stream on bitrates more than 2Mbit/s is transferring.
Finally, AVM has just closed the case with very funny conclusion we excluded at very beginning:
The development department has looked intro your issue once again and comes
to the conclusion that there is no problem on side of the FRITZ!Box. In
your case, please consult your provider for a solution as only he has the
possibility to optimise the behaviour by prioritising VoIP packets on their
routers.
